Community Hospital Integration Project Program (CHIPP)

We provide support for individuals transitioning from State Hospitals to community-based living with clinical and residential assistance. Service Access & Management, Inc provides all referrals to the CHIPP Program.

The CHIPP Program provides a full complement of clinical support, including a psychiatrist, registered nurse, licensed practical nurse, medical assistant, and program coordinator.

Program services are provided in the community in a group home, family home, or supported independent apartment and are underpinned by recovery principles. These principles align with our organizational commitment to person-centered care, meaning that individuals have agency over all aspects of their treatment, including support staff, and promote the individual's dignity and achievements within the community.
